First Time Applicants
(Adults 16 years of age or older)
First time applicants must complete an Application for a U.S. Passport (Form DS-11). Forms are available online through the US State Department website and in person at the MSU Passport Acceptance Facility.
Notes:
- Application should be completed prior to your appointment with the MSU Passport Acceptance Facility.
- Youths 16 or 17 years old must apply in person with one parent.
- Use BLACK INK only to fill in forms
- Please print out the form as two pages; NOT a front-and-back copy. Each page has a barcode at the bottom that must be visible.
Submit the following documents with the completed Form DS-11:
- Proof of United States Citizenship (with the following information is required)
Note: All birth records must meet U.S. Department of State guidelines.- Original State-issued Birth Certificate with raised, colored, or embossed seal
- Certified copy of State-issued birth certificate with raised, colored, or embossed
seal
- Photocopies of birth certificates are not acceptable
- Original Social Security Card
- Birth certificates may be obtained at http://www.cdc.gov/nchs/w2w.htm
- U.S. Citizens born abroad may submit one of the following:
- Original Certificate of Naturalization (Adults 18 or older)
- Original Form FS 240, Consular Report of Birth Abroad
- Original Form DS-1350, Certification of a Report of Birth Abroad
- Original Certificate of Citizenship
- Applicants may be required to submit the following in addition to proof of citizenship: Original name change documents (marriage license, divorce decree, adoption papers)
- Identity Verification
- Current state-issued driver's license

- Driver's license must be the original card. Photocopies and temporarary licenses cannot be accepted.
- Current state-issued ID Card
- Current Military ID

Minors aged 16 or 17 are not required to present photo ID if the accompanying parent has a valid current ID, such as current adult driver license, valid adult passport, or Military ID. If the applicant has a valid ID it should be presented at the time of application.
- Photocopy of ID front and back (Copies can be made at MSU Passport Acceptance Facility)
- Passport Photo: One recent passport photograph meeting passport office requirements, taken within the last six months. Passport photo services can be provided by MSU Passport Acceptance Facility for $15.00.
- Two Required Payments
- 1st payment: Payable to U.S. Department of State (Check, money order, or cashiers check only.)

Execution/processing fee ($35.00)

NOTE: Additional ID Alternatives:
- Out-of-state driver's license or enhanced driver's license with photo
- Learner's or temporary driver's permit (without a photo)
- In-state, fully valid non-driver ID (without a photo)
- Out-of-state, non-driver ID
- Temporary driver's license (without a photo)
- Social Security card
- Voter registration card
- Employee work ID
- Student ID
- School yearbook with identifiable photograph
- Selective Service (draft) card
- Medicare or other health card
- Expired driver's license
- Form DS-71, for an Identifying Witness (only available at your local Acceptance Facility or a Passport Agency).
